4-star Recognition For Chase Memorial Sparks Community Celebration

On November 9th, 2025, Chase Memorial Nursing Home and Rehab announced a Town Hall Celebration to honor the facility achieving 4 stars in Quality Measures from the Centers for Medicare & Medicaid Services, in recognition of its commitment to clinical excellence and superior resident care.

The celebration will be on 2 p.m. on Tuesday, November 12th, at the Chase Memorial Nursing Home and Rehab in New Berlin. According to the announcement, these ratings are based on data submitted by the facility through regular reporting cycles, including clinical outcomes, staffing levels, and health inspection results.

In the release, it was noted that the Quality Measures rating evaluates outcomes related to how well a facility supports residents' physical and clinical well-being, including reducing hospital readmissions, preventing falls, and improving overall health management.

The improved quality score for Chase Memorial is due to initiatives that improve care coordination, reduce hospital readmissions, and strengthen staff training. Also, Chase Memorial recently advanced to an overall 3-star rating.
